If you find that you don’t qualify for a Self employed health insurance plan, the next option that you may want to consider is an Individual health insurance plan. Contact one of the insurance companies listed below for an application kit or if you need any additional information. Rates may vary depending upon the month in which you enroll. To verify the rates listed below, please call applicable HMO directly. Each rate table below contains three columns from left to right. The first column contains the type of coverage; the second column contains the monthly premium rates for HMO plan and the third column contains the monthly premium rates for Point of Service Plan(POS). |
